Key message: many people in remission can fast safely with adjusted medicine times. The decision is individual and is made with your doctor 4–6 weeks before Ramadan, and Islam permits the sick not to fast when fasting would harm them.
Is fasting suitable for me?
| Situation | Usual advice |
|---|
| Disease quiet for months, on once-daily tablets, injections or infusions | Fasting is usually possible, with follow-up |
| Mild activity, steroids being tapered, a pouch or stoma with normal output, a narrowing, diabetes as well, older age, or pregnancy or breastfeeding | Needs individual assessment; fasting on some days only, or not fasting, may be advised |
| Moderate or severe flare, recent admission or surgery, high-output stoma or short bowel, dehydration or kidney problems, marked weight loss or severe anaemia, or pregnancy with active disease | Fasting is not advised |
Medicine times (after agreeing with your doctor)
- Mesalazine: the whole daily dose can be taken once with iftar or suhoor, or split between them Proven
- Azathioprine and mercaptopurine: once daily at the same time (iftar or suhoor).
- Prednisolone and budesonide: with suhoor, because a morning dose suits them best.
- Twice-daily tablets (such as tofacitinib): one with suhoor and one with iftar; once-daily tablets (such as upadacitinib) at the same time each day.
- Weekly methotrexate and injections under the skin: can be taken after iftar if you prefer.
- Intravenous infusions: arrange the appointment with the clinic in advance, and do not delay a dose beyond what your doctor allows.
- Suppositories and enemas: at bedtime after iftar.
- For religious questions about injections and infusions, you can ask a religious scholar.
- Do not stop or delay any medicine because of fasting; a relapse from stopping treatment is far more harmful.
Food and fluids
- Iftar: start with water, one or two dates and soup, then a moderate meal after prayer rather than one large meal.
- Suhoor: as late as possible, rich in protein and slow carbohydrates (eggs, laban, oats, brown bread), with water.
- Drink 2–3 litres spread between iftar and suhoor, not all at once.
- Limit fried food and Ramadan sweets (luqaimat, samosa), and a lot of karak or coffee, which increase fluid loss.
- With a narrowing: smaller meals, chew well, and avoid coarse fibre.
- With a pouch or stoma: oral rehydration solution after iftar, extra salt with food, and monitor output.
- At Eid: return to normal eating gradually and avoid back-to-back heavy feasts.
Break your fast and contact the clinic if:
diarrhoea increases or you see blood, you have severe pain, vomiting or fever, signs of dehydration (dizziness, severe thirst, small amounts of dark urine), a clear rise in stoma output, or low sugar if you have diabetes.
